Case Specialist, Health Promotion - Boston, MA

The mission of the International Institute of New England (IINE) is to create opportunities for refugees and immigrants to succeed through resettlement, education, career advancement and pathways to citizenship. One of the oldest and largest human services organizations for New Americans in the region, IINE serves more than 4,000 people each year in Boston and Lowell, Massachusetts, and Manchester, New Hampshire. Our programs include case management, youth and family services, English for Speakers of Other Languages (ESOL), employment and skills training, and immigration legal services.

Overview
The Health Promotion Case Specialist supports the resettlement and integration of vulnerable refugee
families in Massachusetts through the provision of extended strengths-based case management related
to health care and reducing barriers to integration. The Health Promotion Case Specialist supports
recently-resettled refugee students with their enrollment and engagement in health care services through
health care education. The Health Promotion Case Specialist will provide Domestic Violence education,
support developing safety plans, and coordinate with Lowell Health care providers to ensure refugee
clients have access to services. The Health Promotion Case Specialist will also support enrolling Haitian
Entrants in IINE services and provide health care case management for Haitian clients.

Responsibilities

  • Provide comprehensive case management services tailored to clients’ unique goals.
  • Conduct enrollments and needs assessments with new clients; facilitate development of individualized, mutually-agreed upon goals and care plans.
  • Develop health care and safety plans for clients
  • Provide workshops and trainings related to Health Care and Domestic Violence
  • Build, develop and support relationships with Health Care providers and Domestic Violence providers
  • Assess clients’ progress on self-sufficiency goals at intake, mid-enrollment, and completion of services; report on clients’ progress through online database.
  • Coordinate clients’ care with relevant medical and social service providers; plan, implement, and evaluate treatment options and services to promote quality and cost-effective physical and mental health outcomes for each client.
  • Facilitate referrals to in-house and community-based services, including public benefits, employment services, career readiness opportunities, and legal services.
  • Support and expand the Institute’s capacity to provide intensive case management services to
  • refugees with special needs.
  • Coordinate closely with other Institute programs and services to enhance service delivery.
  • Promote family self-sufficiency and wellness; develop safety plans and support clients with crisis management services as necessary.
  • Ensure school-aged children are enrolled in school; facilitate the engagement of children in
  • school and extracurricular activities.
  • Provide orientation to recently-resettled parents about U.S. school systems.
  • Keep professional and accurate documentation in online databases; maintain client case file and case notes; contributed to monthly/quarterly/annual reporting as assigned.
  • Provide community outreach to strengthen the capacity of local service providers, employers, community/religious organizations, and citizen volunteers to be culturally responsive to new refugee groups and supportive of their integration into American society.
  • Assess clients’ progress on self-sufficiency goals regularly.
  • Participate in monthly network calls or training opportunities.
  • Attend staff meetings.
  • Perform all other duties assigned by supervisor.

Required Skills/Abilities

  • Ability to work with diverse clients requiring a range of support services.
  • Driver’s license and access to a vehicle.
  • Interest in international, refugee, and asylee issues.
  • Ability to work as part of a team and independently.
  • Excellent communication and advocacy skills.
  • Extremely well-organized, detail-oriented, flexible, and able to manage multiple tasks
  • simultaneously.
  • A command of Microsoft Word, Excel, Outlook, and PowerPoint.
  • High-levels of discretion and professionalism in dealing with confidential information.
  • Haitian Creole, French, and Spanish fluency preferred.

Education & Experience

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
  • Minimum of two years post-degree social work or case management experience.
  • Experience working in academic, legal, and public benefit systems preferred.

Work Environment Conditions

  • Exposure to bright, overhead office lighting, use of office technology such as laptops, printers, mail machines, and other in-office equipment.
  • Occasional Driving to/from client homes.
  • Frequent interruptions from employees and clients- must have ability to multi-task

Physical Demands

  • Occasional lifting, pushing, and pulling materials (boxes, equipment) of 50 pounds or more.
  • Continuous sitting (or standing) at a desk for long periods of time.
  • Occasional driving to and from IINE offices as needed.

Covid-19 Policy

IINE does not require applicants for most positions to show proof of Covid-19. Some federal contracts, however, require IINE staff to show proof of Covid-19 vaccination, and some immigration status adjustments, such as a Green Card, require proof of Covid-19 vaccination as well. Consequently, for some positions, IINE staff may be required to show proof of vaccination, and in others, IINE staff will be supporting clients who are required by law to have proof of vaccination in order to adjust their status in the United States.
